Tonsillitis is a common medical condition characterized by the inflammation of the tonsils, which are two small masses of lymphoid tissue located at the back of the throat, one on each side. This condition can affect people of all ages, but it is most frequently seen in children and adolescents.

The primary causes of tonsillitis are viral or bacterial infections. Viruses like the Epstein-Barr virus and Adenovirus are often responsible for viral tonsillitis, while Streptococcus pyogenes, commonly known as Group A Streptococcus, is the most common bacterium causing bacterial tonsillitis, commonly referred to as strep throat.

The hallmark symptoms of tonsillitis include a sore throat, difficulty swallowing, and swollen tonsils. Patients may also experience other symptoms such as fever, headache, and a scratchy or muffled voice. In some cases, there may be white or yellow patches on the tonsils or in the throat.

Diagnosing tonsillitis usually involves a physical examination by a healthcare provider and, in some cases, a throat swab to determine the specific cause, especially if streptococcal infection is suspected.

Treatment for tonsillitis varies depending on its cause. If the condition is viral, it typically resolves on its own with rest, fluids, and over-the-counter pain relievers to manage discomfort. However, if the cause is bacterial, antibiotics, such as penicillin, may be prescribed to eliminate the infection and reduce the risk of complications.

In recurrent or severe cases of tonsillitis, where the tonsils become chronically infected or enlarged, a healthcare provider may recommend a tonsillectomy, which is the surgical removal of the tonsils. This procedure is more commonly performed in children but can be necessary in adults as well.

In conclusion, tonsillitis is a common throat condition caused by viral or bacterial infections. It presents with symptoms like sore throat, difficulty swallowing, and fever. Hydrocephalus, also called water in the brain, is a condition where there is an abnormal build-up of CSF (cerebrospinal fluid) in the cavities (ventricles) of the brain. The build-up is often caused by an obstruction that prevents proper fluid drainage. The build-up is often caused by an obstruction that prevents proper fluid drainage.

In hydrocephalus, the build-up of CSF can raise pressure inside the skull, which squashes surrounding the brain tissue.

In some cases, this can cause the head to steadily grow in size, convulsions, and brain damage. Hydrocephalus can be fatal if left untreated.

Other symptoms include headache, vomiting, blurred vision, cognitive problems and walking difficulties.

Symptoms

Symptoms of congenital hydrocephalus 

  • breathing difficulties
  • arm and leg muscles may be stiff and prone to contractions
  • some developmental stages may be delayed, such as sitting up or crawling
  • the fontanelle, the soft spot on the top of the head, is tense and bulges outward
  • irritability, drowsiness or both
  • unwillingness to bend or move the neck or head
  • poor feeding
  • the head seems larger than it should be
  • the scalp is thin and shiny and there may be visible veins on the scalp
  • pupils of the eyes may be close to the bottom of the eyelid, sometimes known as “the setting sun”
  • there may be a high-pitched cry
  • possible seizures

Causes

  • Too much CSF is produced.
  • One of the ventricles in the brain is blocked or narrowed, stopping or restricting the flow of CSF, so that it cannot leave the brain.
  • CSF cannot filter into the bloodstream.

Treatment

Hydrocephalus requires urgent treatment to reduce the pressure on the brain; otherwise, there is a serious risk of damage to the brainstem, which regulates functions such as our breathing and heartbeat.

A shunt is the surgical insertion of a drainage system. A catheter (a thin tube with a valve) is placed in the brain to drain away excess fluid from the brain into another part of the body, such as the abdomen, the chest cavity, or a chamber of the heart. Usually, this is all that is needed, and no further treatment is required.

Patients with hydrocephalus will usually need to have a shunt system in place for the rest of their lives. If the shunt is placed in a child, additional surgeries may be needed to insert longer tubing as they grow.

Ventriculostomy – the surgeon makes a hole in the bottom of a ventricle so that the excess fluid flows towards the base of the brain. Normal absorption occurs at the base of the brain. This procedure is sometimes performed when the flow of fluids between ventricles is obstructed. Read More

Stress is a feeling of emotional or physical tension. It can come from any event or thought that makes you frustrated, angry or nervous.

Hungarian-born Hans Selye (1907-1982) stumbled upon the idea of the General Adaptation Syndrome (G.A.S.). The G.A.S., alternately known as the stress.

         There are two main types of stress:

Contrast Media is a chemical substance of very high or very low atomic number or weight, therefore it increases or decreases the density of the organ under examination.

Or

A substance which when introduced into the body will increase the radiographic contrast in an area where it was absent or low before.

Contrast procedures have been developed to increase the native contrast of organs in order to separate them from surrounding tissues. There are many situations where they can be a useful supplement to the information gained from routine radiographic examination. They can provide information on the size, shape and position of structures. They might help outline the internal structure of an organ, including its mucosal pattern and sometimes help assess its function.

What is an ideal contrast media?

An ideal contrast is which is Easy to administer, High water solubility, No toxicity, Stable Compound, Concentrate in area of interest, Should have rapid elimination and Cost effective.

What are the modes of administration?

Radiological contrast can be administer through Orally, Rectally, Intravenously – (injection/ infusion), mechanically – Filling of a body cavity or potential space and Intra-muscularly.

What are the uses Of Contrast Media?

They are used for many radiological procedures like Arteriography, Angiography (DSA) – Cardiology, Venography (replaced by ultrasound Doppler), IVU, Fluoroscopy – Alimentary tract, HSG, sialography, CT, MRI, Ultrasound – Liver, kidney, Myelography (replaced by MRI) andArthrography – Knee joints.

Classification of Contrast Media: –

X-Ray & CT Contrast are classified toPositive CMand Negative CM.

Positive contrast agents have a high atomic number, either barium sulphate or iodine and appear more radiopaque than the surrounding tissue and appears white on radiographic film. Barium sulphate used for digestive system abnormalities and Iodinated contrast medium used for all the parts like angiography, myelography, HSG etc of body as it is watery soluble contrast media and easily excreted from the body.

Negative contrast agents are gases of low density like air which appear radiolucent. Low atomic number material and appears black on film. Examples are Carbon dioxide, Oxygen and Air.

There are a few conditions like the integrity of gut wall compromised or GI Perforation, previous allergic reactions to barium, and suspected fistula between esophagus and lung in which the barium sulfate should not use for diagnostic purposes.

For iodinated contrast media, contraindications are History of Allergies, Pregnancy, Renal Failure, Asthma, Diabetes, and Cardiac Diseases. The contrast should avoid for such conditions because it can be dangerous to the patient’s health and life.

Adverse reactions of iodinated contrast media: – 

In clinical medicine, every technical procedure and every drug is given to a patient by whatever route (oral, rectal, IV, IA, intrathecal) may cause adverse drug reactions (ADRs), which are sometimes fatal. It is inevitable that even when administered appropriately, RCM will very occasionally result in a severe or even fatal ADR, sometimes leading to litigation.

It has become increasingly difficult to defend inappropriate indications, inadequate resuscitation facilities, and inappropriate experience, especially in small imaging departments, in either private or public hospitals. Adverse reactions to RCM may be divided into:

1. Idiosyncratic anaphylactic reactions

2. Non-idiosyncratic reactions and 

3. Combined reactions. These reactions can be minor, moderate and severe and their effects to the patients.

Treatment for adverse reactions: –

If the patient is having minor effects then there is no need for treatment only attention and relaxing to the patient is required, if the patient is having moderate reaction then there is no need to hospitalize the patient but required treatment immediately and if patient is having severe reaction the patient should shift to the hospital immediately for appropriate treatment.

Aspergillosis is an infection or allergic response due to the Aspergillus fungus.

The fungus is often found growing on dead leaves, stored grain and compost piles or in other decaying vegetation. 

There are several forms of Aspergillosis:

1. Allergic pulmonary Aspergillosis

Allergic pulmonary Aspergillosisis an allergic reaction to the fungus. This infection usually develops in people who already have lung problems such as asthma or cystic fibrosis.

2. Aspergilloma

Aspergillomais a growth (fungus ball) that develops in an area of past lung disease or lung scarring such as tuberculosis or lung abscess.

3. Invasive pulmonary Aspergillosis

Invasive pulmonary Aspergillosisis a serious infection with pneumonia. It can spread to other parts of the body. This infection occurs most often in people with a weakened immune system. 

This can be from cancer, AIDS, leukaemia, organ transplant, chemotherapy drugs that lower the number, function of white blood cells or weaken the immune system.

SYMPTOMS:-

Symptoms depend on the type of infection.Symptoms of allergic pulmonary Aspergillosis may include:

  • Cough
  • Coughing up blood or brownish mucus plugs
  • Fever
  • General ill feeling (malaise)
  • Wheezing
  • Weight loss

Other symptoms depend on the part of the body affected, and may include:

  • Bone pain
  • Chest pain
  • Chills
  • Decreased urine output
  • Headaches
  • Increased phlegm production, which may be bloody
  • Shortness of breath
  • Skin sores (lesions)
  • Vision problems

DIAGNOSIS

Tests to diagnose Aspergillus infection include:

  • Complete blood count
  • Galactomannan (a molecule from the fungus that is sometimes found in the blood)
  • Immunoglobulin E (IgE) blood level
  • Lung function tests
  • Sputum stain and culture for Aspergillus

Treatment

A fungus ball is usually not treated with antifungal medicines unless there is bleeding into the lung tissue. In such a case, surgery and medicines are needed. Invasive Aspergillosis is treated with several weeks of an antifungal medicine.

What is anemia?

Anemia results from a lack of red blood cells or dysfunctional red blood cells in the body. This leads to reduced oxygen flow to the organs.

Symptoms may include fatigue, skin pallor and shortness of breath, light-headedness, dizziness or a fast heartbeat.

Treatment: Vitamin B12 supplements and dietary changes (eating foods rich in vitamin B12, such meat; fish; eggs; dairy products; and breads, cereals, and other foods fortified with vitamin B12)

Aplastic Anaemia

The term “anemia” usually refers to a condition in which your blood has a lower than normal number of RBCs. However, some types of anemia, such as aplastic anemia, cause lower than normal numbers of other blood cells, too. Aplastic anemia can occur if your bone marrow is damaged and can’t make enough RBCs, WBCs, and platelets. 

The causes of aplastic anemia can be acquired or inherited. Groups at risk People undergoing radiation or chemotherapy, exposed to toxins, or taking certain medicines.

Treatment:

 Depends on the cause of the anemia. Treatments may include blood transfusions, medicines, blood and marrow stem cell transplants, and lifestyle changes. 

Haemolytic Anaemia:

 Normally, RBCs have a lifespan of about 120 days. Your body constantly makes new RBCs to replace ones that die. Sometimes, RBCs are destroyed before their normal lifespan is up. Hemolytic anemia occurs if your body can’t make enough RBCs to replace those destroyed. Acquired hemolytic anemia occurs if your body gets a signal to destroy RBCs even though they are normal. Inherited hemolytic anemia is related to problems with the genes that control RBCs. 

Treatment: Depends on the cause of the anemia. Treatments may include blood transfusions, medicines, surgery and lifestyle changes.

Sickle Cell Anaemia:

Sickle cell anemia, an inherited group of disorders, red blood cells contort into a sickle shape. The cells die early, leaving a shortage of healthy red blood cells (sickle cell anemia) and can block blood flow causing pain (sickle cell crisis). Read More

But things got a lot harder last year, didn’t they? Suddenly there as an unprecedented situation with COVID-19 that put you front and center. To make matters worse, it was an election year in a very divided country. While you continued to try to remain above the fray, politicians, pundits, and even the common person on Facebook had a very strong opinion about how you were operating, what you were saying, and how you were (or weren’t) leading.  Read More

I know it was a difficult circumstance. People don’t realize that science is a process, and we were all faced with a threat about which we knew very little. Do we wear masks? Do we socially distance? Do we close schools? What treatments are effective, what treatments are ineffective, and what treatments are unclear and need further study. I realize science works that way, with recommendations changing and often contradicting each other (it’s been that way ever since I became a doctor). But for most people this past year was their first glimpse into the often unclear nature of the scientific process. We go from a point of not knowing and accumulate data to come up with best answers. But many times those best answers are met with new data that contradicts and causes those best answers to need changing. You did your best. The politicians and pundits (not to mention the attention-seeking “experts” on YouTube and Facebook) are largely to blame for the confusion, and certainly they didn’t accept science for what it is. They wanted the “right” answers now! That’s not how it works, and I realize that.

So I approach this with great caution. I want you to know how much I like and respect you, and feel like you’ve done a pretty good job, considering just how horrible the political stuff has been around you.

But I think you are getting the vaccine all wrong.

No, I’m not saying we shouldn’t give it. I think the development of the vaccine so rapidly is actually one of the greatest public health accomplishments ever…of all time. Especially with the incredible efficacy of the available vaccines. You surpassed our wildest dreams in that.

But people are still scared of the vaccine, and some are resisting it for “political” reasons. One survey said that 1/3 of Republicans were not going to get the vaccine. We need them to get it. We need to change their mind. While your recent loosening of mask requirements for vaccinated adults was a step in the right direction, I think you are being too cautious. You are considering the risk of vaccinated adults still spreading the virus and not considering the risk of all those people who are too scared (or politically deluded) to get the vaccine. You need to be more bold. You need to up the ante. You need to do what’s best for the health of everyone, which is clearly to get them all vaccinated. Read More

Basically Digital radiography is an advanced form of x-ray examination which uses a computer to form the images. This technique uses x-ray sensitive detectors to capture data during examination, which is immediately transferred to a computer without the use of an intermediate cassette. Digital radiography is advantageous than a conventional x-ray technique because the images can be edited, transferred, stored or manipulated etc.

Using digital radiography the image can be zoom or magnified. Any tilt of the image can be corrected. The contrast and brightness of the image can be corrected accordingly. CR System: –Computed radiography was the first digital technique introduced in radiology department which uses the x-ray recording panels on cassettes instead of radiographic film. The cassette is placed in a reader to convert the recorded information of the patient into a digital image and then the image is displayed on monitor and stored in computer as well.

DR System: –Direct radiography or digital radiography is the latest advancement in radiology department in which the use of any kind of cassette has been completely replaced by x-ray sensitive detectors. The detectors capture the radiation and convert those information directly into an electronic signal with the help of CCD (Charged coupling device) and then transfer the information to computer. The image is directly shown on monitor of computer and stored in storage also.
